1. Download Adobe Acrobat Reader if it is not already installed on your computer.  The software is FREE and it will allow you to view and print the Application since it is in PDF (Portable Document Format).
   
   
2. Click on a link below to view the application form you need:

   2. Certificate Program Application Form (PDF)
      
      
3. Print it out.
   
   
4. Complete the Application and mail it with your application fee to:

• Applications for Admissions to an Associate Degree Program:  $50.00
• Applications for Admissions to a Certificate Program:  $25.00
